Service Description
Evaluation and management of medical conditions for new clients. May require a complete medical history, medication review, and physical exam. Required for enrollment in our weight management and hormone replacement programs. $50 deposit will be subtracted from the visit cost but is non-refundable if canceling or rescheduling without 48 hour notice.
Cancellation Policy
Please be courteous and inform us no less than 48 hours prior to your appointment if you need to cancel or re-schedule. Failure to cancel within 48 hours will require 50% of your services to be paid. Failure to cancel within 24 hours will require your service to be paid for in full! Clients with 2 or more appointment cancellations or 2 or more no-show appointments may be discharged from our practice. If arriving late for your appointment (over 15 minutes) your appointment may need to be rescheduled for another date/time and you may be charged 50% of your scheduled service. Thank you! You can receive a 4% discount if you pay your service in cash.
